Adult Entertainment Application Requirements

• Applications will be processed at Consumer Affairs Division Monday through Friday between the hours of 8:00AM - 4:00PM - NO EXCEPTIONS. Appointments are not necessary.

• Proof of valid identification is required. The following documents are acceptable for this purpose:

• Original Valid Passport with Date of Birth
• Original State Driver’s  License
• Original State Identification Card
• Other Original Photo Identification with Date of Birth

Fees

• New OR Renewal Adult Entertainment Work Identification Card - $100.00

• Duplicate Adult Entertainment Work Identification Card - $30.00

• Payments may be made ONLY by Visa, MasterCard, Discover Card, or
  Money Order.

• Cash and personal checks are  NOT accepted.
