The Company Approved Signature Form must be received by Consumer Affairs before we can accept affidavits from the Caregiver Company/License Holder or applicant. The notarized and signed affidavit will be accepted from the applicant along with their completed Caregiver ID Badge Application and required picture ID.
The Affidavit may be provided to Consumer Affairs by the applicant when they apply for the Caregiver ID badge. The Caregiver Company/License Holder is no longer required to submit the affidavit to our office prior to receiving the Caregiver application.
