Agencies and private individuals can verify Caregiver ID Badges here. You may search for a badge by applicant name, I.D. Badge number, or agency affiliation.
Note to Home Care Businesses:
The results of the Agency search feature indicate Home Caregivers who, at the time of applying for an ID Badge, provided the Home Caregiver Company Endorsement / Affidavit from your agency/business stating that the caregiver successfully completed a level 2 fingerprint-based criminal history records check and that if the affiliation with your agency changes, Consumer Affairs will be notified. The results are not intended to reflect the entire human resource staff compliment of each home care business / agency since many caregivers work for multiple agencies and may be covered under a separate Home Caregiver Company Endorsement/Affidavit. If a Home Caregiver's name appears on your agency's list and they are no longer affiliated, submit the Home Caregiver Company Update Form by e-mail to firstname.lastname@example.org or fax to (561) 712-6610.
