APPLICATIONS:

Applications will be processed at Consumer Affairs Division Monday through Friday between the hours of 8:00AM - 4:00PM - NO EXCEPTIONS.
Appointments are not necessary.

Incomplete application packets will not be accepted.

Proof of valid identification is required. The following documents are acceptable for this purpose:

  • Original Valid Passport with Date of Birth
  • Original State Driver's  License
  • Original State Identification Card
  • Other Original Photo Identification with Date of Birth
