Case Managers assure full coordination and linkage of services. Case Managers will assist with applications for food stamps and other needed services available in the community. 

Case Managers involve, with client’s consent, family and caregivers in the client’s plan of care. Case Managers assist families and caregivers to resolve the emotional, financial and physical demands of caring for their elder loved ones.  

Most homebound seniors in need of long-term care services prefer to get help in their home. Experienced Case Managers provide assessments to determine senior’s individual needs and coordinate and manage in-home services.

DOSS provides professional home visits.  Our caring Case Managers will accommodate your schedule to provide the most convenient time for you.  Home visits are conducted to review existing services and arrange additional in home assistance if needed.
