If it has been determined that an exposure has occurred, the exposed employee’s Designated Infection Control Officer or their designee shall complete Section 3 ("Form of Request") of the worksheet and forward to the emergency room physician where the source patient is being treated.

The emergency room physician completes Section 4 ("Evaluation and Response to Form of Request") of the worksheet.
